Welcome to our survey about the GP to CPCS service. 

A new national scheme has been introduced to help you get the medical help you need from the right professional at the right time. Your GP surgery has joined this national scheme to provide a new referral pathway for you called GP to Community Pharmacy Consultation Service (CPCS). This is where your GP practice may refer you to your local pharmacist for advice and consultation.
They will only do this with your agreement, and if your illness/condition is appropriate. Pharmacies are open for longer and at weekends and therefore they can provide a consultation for you  more quickly either over the phone, video call or in the pharmacy. Community pharmacists are highly qualified professionals with expertise in medicines. 

To help us to continue to develop and improve this service, we are keen to find out more about your experience and how you found the service. 

We would really appreciate it if you could share your thoughts with us and answer a few questions in this survey which will only take a few minutes of your time.
