Our local NHS is working with partners across Humber and North Yorkshire to improve the heart health of our community. 

This survey will help to capture your thoughts, views and experience of:

· What you know about cardiovascular disease.

· What stops you from accessing health and care services for you and others in your community.

· What would help you to access health and care services.

Key words to help you:

Cardiovascular Disease (CVD): A class of diseases that involve the heart or blood vessels, including coronary artery disease (CAD), heart attacks, stroke, and hypertension (high blood pressure). 

Hypertension (High Blood Pressure): A condition where the force of blood against the artery walls is too high, increasing the risk of heart disease and stroke. 

Cholesterol: A waxy substance found in the blood. High cholesterol can block your blood vessels. It makes you more likely to have heart problems or a stroke.
