Health and wellbeing survey
Healthwatch Surrey is seeking the views and experiences of 50-66 year old Surrey residents about what they do to maintain their own health and wellbeing. The results of this will help to shape local NHS and Surrey County Council services.
Healthwatch Surrey champions the voice of local people to shape, improve and get the best from NHS, health and social care services. We are independent and have statutory powers to make sure decision makers listen to the experiences of local people.
Responses to this survey are anonymous. The closing date for the survey is 31January 2026.
3 people who complete the survey before 31January 2026 will be selected at random to receive a £20 Love2shop voucher. Those selected will be notified within 1 month of the close of the survey.
It will take about 20 minutes to complete.
