Welcome to the survey start page. Ofsted and the Care Quality Commission (CQC) are visiting your area to understand more about how local area partners work together make sure that they are providing the right services for children and young people with special educational needs and/or disabilities (SEND). Thank you for helping us by answering this survey.  
   
As part of the visits, inspectors want to know more about whether children and young people with SEND have access to education, health and social care services that meet their needs. 
 
This survey is for parents and carers of children and young people with SEND only.

This survey is optional; you do not have to do it if you do not want to. It should take around 5 to 10 minutes to complete. Your answers will not contribute to any judgements of your local area. We are not asking for any details that may identify you. Please do not give personal information that may identify you or others in your answers. 
 
We will use the evidence we collect through surveys to help inform our national report that will be published online. We may use it as part of inspector's evidence collection while they are on the visit to your local area. We will not write anything that can identify you.  
 
If you are both a parent/carer and a professional working with children and young people with SEND, you are welcome to complete both the parent/carer and the professional survey from alternative perspectives.

Your information  
This survey is run by SmartSurvey. You can find out what they will do with your information here: www.smartsurvey.co.uk/privacy-policy 
 
SmartSurvey will pass your information to Ofsted, and Ofsted will delete the information 3 years after the final national report is published. However, we may use the information for larger pieces of research to help local area services improve or other statistical research. If that happens, we may keep the information until the research is completed. 
 
Once you have submitted your survey you will be unable to make changes or remove it. This is because it is anonymous so we will not be able to identify your survey.

Ofsted uses machine learning to find themes in the survey answers.  
 
Machine learning gives data (information) to a computer so it can learn what to do by itself without being told. 
 
 
Safeguarding concerns  
Our surveys are anonymous, but if what you tell us makes us worried about you or that a child or young person is at risk of harm, we will tell the local authority.  
 
If you think a child or young person is at risk of being abused or neglected, contact the children’s social care team at your local council. Further information about how to report child abuse to a local council can be found here.  You can also contact the NSPCC on 0808 800 5000 or here. 
 
For young adults over the age of 18, local authorities have adult safeguarding teams. If you are concerned about a young adult, call your local council and ask to speak to the adult safeguarding team or look on the local adult safeguarding partnership website where the details of who to contact should be available.
