This five-minute survey, run by Healthwatch Brighton and Hove, is to hear people's views and experiences about their health and wellbeing.
Healthwatch Brighton and Hove helps to improve health and social care services across the area by listening to people, and using this to influence people who design services. We are completely independent of the NHS and the Council.
This survey will include looking at ways we can help people to use health care services when they need to.
At the end of the questionnaire, there is an opportunity to enter a prize draw to win one of three £20 High-Street vouchers - you can sign up to this in the last question.
We would also like to hear more about your views - this is completely voluntary and would be a phone or online conversation - more details about this towards the end of the survey.
