This survey is about adjustments (changes) your GP practice makes so you can use their services. In this survey, adjustments include reasonable adjustments described in the Equality Act 2010 as a change that a service provider must make to remove or reduce barriers faced by a disabled person. It also means language translation which is a right when using health services for people who use English as a second or additional language.

 

Healthwatch Surrey champions the voice of local people to shape, improve and get the best from NHS, health and social care services. We are independent and have statutory powers to make sure decision makers listen to the experiences of local people.

 

Healthwatch Surrey is seeking your experiences of adjustments,  whether you have asked for them to be put in place to help you use services at your GP practice and how well your GP practice is meeting your needs.

 

Responses to this survey are anonymous. The closing date for the survey is 31 December. 

 

5 people who complete the survey before (31 December) will be selected at random to receive a £10 "One 4 All" gift card. Those selected will be notified within 1 month of the close of the survey.

 

There are 14 questions (plus 12 optional demographic questions), and it will take about 15 minutes to complete.
