Mental Health: Staying Well After Discharge

Healthwatch Staffordshire is interested in understanding how NHS services support you during your care to help you stay well after you're discharged.

Please only take part in this survey if you:

  • Are 18 years old or over
  • Live in Staffordshire (excluding Stoke-on-Trent)

The information we collect will be anonymous. The feedback will be analysed and published in a report. We may use quotes in our reports, but we will not use any information that will identify you. The report will be shared with organisations who run and commission these services in order to understand any problems and how the service could be improved.  If you are sharing information on behalf of another person, make sure that you have their permission to do so.

We will also ask you to volunteer some personal information to understand how different groups experience local health and social care services. This supports our focus on improving equality, diversity and inclusion.
