Healthwatch Trafford are interested in hearing from people receiving palliative care, and family members or carers of those who are receiving palliative care. We would also like to hear from family members or carers of people who were receiving palliative care, but have passed away within the last 2 years. Please only fill in the survey if the palliative care is or was being received in Trafford.
Palliative care is defined as ‘practical and emotional support for those with a life limiting illness which can be provided in a range of settings from home to hospice care.’
This can be a difficult topic to share detail of, and we appreciate the time and effort taken to complete this survey. By completing this survey, you will help us provide recommendations which will help to improve palliative care services in Trafford. Signposting information is included at the end of the survey for services which can provide support.
Our findings will be made available online. Where relevant, the report will be shared with Trafford Council and the local Trafford Integrated Care Partnership (ICP).
It would be very helpful to us if you could complete the demographic information section at the end of this survey as this helps us to monitor how representative of the Trafford population the findings of our research are. The information collected will be anonymised, so you will not be identifiable by your answers. Your personal details will never be passed on or supplied to any other organisation, and we will only ever use it for the purposes of this project.
