Live Well Cheshire West is a new website which will help you find local services, support, activities and events for residents in Cheshire West including children who have special educational needs and disabilities (SEND), young people, families and adults.

The site is run jointly by Cheshire West and Chester Council and local NHS partners as part of the Cheshire West Integrated Care Partnership.

We want you to tell us what you think and help us improve our website.  You will need to have the Live Well site open as you complete the survey.  Please open https://livewell.cheshirewestandchester.gov.uk  This survey will take roughly 15-20 minutes.  Thank you for taking the time to give us your feedback. 



Some of the questions contained in this survey relate to your personal situation, this allows us to know who has tested the website and to ensure that people from a diverse range of backgrounds have provided us with their feedback. If you feel as though you do not want to answer a question please select ‘I’d rather not say’. All questionnaires are completely anonymous and therefore there is no way in which your answers can be related back to you unless you indicate otherwise at the end of the form that you would like to discuss it further with us.
