1. Introduction

Your views are really important to us, so thank you for agreeing to complete this short survey. Greater Manchester Health and Social Care Partnership, alongside Bury, Bolton, Manchester, Rochdale, Tameside, Trafford, Oldham, Salford, Stockport and Wigan, have commissioned this survey as part of an independent evaluation of their local care approaches from Cordis Bright, Traverse, PPL and COBIC. Local care approaches include a locality plan, a Local Care Organisation, and integrated commissioning arrangements. They also involve the transformation of a range of health and care services. They are being implemented in the context of the Greater Manchester Strategic Plan for Health and Social Care.

The purpose of this survey is to understand your experience of the local care approach in your locality, and what impact it has had so far. Your responses will remain confidential and anonymous. We will not attribute responses, comments, or your views to you in any report that we produce on the basis of this survey.
