1. Qualifying questions- Information

Thank you for your interest in the online Hope programme. Please take some time to complete this short survey to check your eligibility and to book onto this course.

The purpose of this survey is to assess if you are eligible to participate in the Self-directed online Hope Programme and/or the Facilitated online Hope programme and to provide those eligible with a link to the relevant registration page.

We will ask optional questions about your background, including your age, gender, sexual orientation, religious beliefs, ethnicity and health status. This helps Macmillan Cancer Support know who is taking part in the Hope Programme or if some groups are being left out, so we can try to make the course easier for everyone to join. You don’t have to answer these questions if you don’t want to — just choose ‘prefer not to say’.

Individual responses will not be stored for people who are not eligible to participate in the online Hope programme. Responses for people who are eligible to participate will be deleted when the course has finished. Responses to this survey will be combined and analysed/reported in aggregate. The anonymised reports of the characteristics of people accessing the online Hope programmes will be used indefinitely.

By submitting this form, you consent to us using your details to provide you with access to the online Hope platform.

Your anonymised data will help Macmillan Cancer Support understand if the Hope Programme is accessible to everyone. All of your responses will be kept confidential. We promise to keep your details safe and never sell or swap them with anyone. This includes any sensitive personal data you may provide. Our privacy policy explains how we keep this promise.

If you don’t want to hear from us, or change your mind about how we contact you, please email us at ServiceOpsSupport@macmillan.org.uk - we’re happy to help.
