Mae'r arolwg hwn yn gofyn cwestiynau am eich profiad o asesiad ar gyfer proses gymhwyster Gofal Iechyd Parhaus y GIG. Trwy roi'r adborth hwn i ni, gallwn ganfod cyfleoedd lle gallai fod angen i ni wella a hefyd gael gwybod am ble mae unigolion yn cael profiadau da.

Hynny yw, data sy'n golygu bod modd eich adnabod chi neu rywun arall, fel enwau, cyfeiriad, manylion cyswllt e-bost neu rifau ffôn. Nid yw'r arolwg hwn yn rhan o unrhyw broses gwynion neu apeliadau yn ymwneud â phenderfyniad ar gymhwyster Gofal Iechyd Parhaus y GIG. Os byddwch yn anghytuno gyda'r penderfyniad ar gymhwyster a wnaed neu os bydd gennych unrhyw bryderon, bydd angen i chi gysylltu â'r Tîm Gofal Iechyd Parhaus yn uniongyrchol. Sut i gwblhau'r arolwg
Os ydych chi / yr unigolyn rydych yn ei gynrychioli wedi cael mwy nag un asesiad cymhwyster, dylech gwblhau'r arolwg gan ystyried eich profiad diweddaraf.
Atebwch y cwestiynau isod trwy ddewis yr opsiwn sy'n cyfateb yn fwyaf agos i sut rydych yn teimlo am bob datganiad neu gwestiwn. Gallwch ddefnyddio'r Bocsys Sylwadau o dan bob cwestiwn i roi mwy o wybodaeth i ni am pam rydych wedi dewis eich ateb, os ydych yn awyddus i wneud hynny. This survey asks questions about your experience of the NHS Continuing Healthcare (NHS CHC) assessment for eligibility process. By giving us this feedback, we can identify opportunities where we may need to improve and also find out where individuals are having good experiences.
Taking part in this survey is optional and what you tell us is confidential. Your answers will remain anonymous and only be used for the purposes of improving services. Please DO NOT include any personal data. That is data that identifies you or someone else, such as names, address, contact emails or phone numbers. Should you include any personal data, this will be deleted from the survey response.
This survey does not form part of any complaints or appeals process regarding an NHS CHC eligibility decision. If you disagree with the eligibility decision that has been made or have any concerns, you will need to contact the Continuing Health Care Team directly. How to complete the survey
If you / the person you are representing has undergone more than one assessment of eligibility, you should complete the survey considering your most recent experience.
Please answer the questions below by selecting the option that most closely matches how you feel about each statement or question. You can use the Comment Boxes under each question to give us some more information about why you have selected your answer, if you want to. However, please ensure that you do not include any information that would identify you or others as this is an anonymous survey.
The survey should be completed from your perspective. If you are not the person who has been assessed, and the individual is not able to communicate their views, the answers should reflect your experience as a family member / carer etc. The survey covers only the NHS CHC eligibility assessment process, and not support or care planning.
