Healthwatch Staffordshire would like to hear the experiences of women who reside within STAFFORDSHIRE who have been diagnosed with a gynaecological condition.

Healthwatch Staffordshire has put together this list of questions to help us understand the experiences of women who reside within Staffordshire who have received a diagnosis of a gynaecological condition.

We would like to invite you to take our survey.  You may answer the survey anonymously, you can also give us your contact details at the end if you would like to share your story as part of a case study or to discuss anything further. We will not use your details for any other purposes.

The views collected by the survey may be included in one of our Reports or used in our own statistics.  Your name will not be attached to specific comments in our reporting, unless you ask us to do so.

We will also ask you to volunteer some personal information to understand how different groups experience local health and social care services.  This supports our focus on improving equality, diversity and inclusion.
