Betsi Cadwaldr University Health Board is your local NHS. We provide the health services that you, your family and friends will recognise in your local area such as GPs, dentists and hospitals.

It is important for us to understand how you feel we are doing in terms of listening and public engagement.  Your views help us to improve and learn about the issues that matter to you.

To do that well we need you to be honest with us and tell us what works and what needs improving. 

This survey will take approximately 15 minutes to complete, and all your responses will be kept strictly confidential and used only for the purpose of this engagement exercise. 

The questionnaire is voluntary, anonymous and confidential, all feedback received will be logged, reviewed and key themes identified. 

The deadline for completing the questionnaire is Friday 19 September.  Thank you for your time and valuable contribution.

Please ensure that when you complete free text boxes, you do not enter any information, which may potentially identify you or any other individual.  The information you provide will be used to develop our proposals and kept in line with Data Protection legislation.
