This survey is designed to check if you have completed key pre-transition requirements for items associated with the Employer Portal. You will need to access the Employer Portal even if you have a shared service or payroll provider that works on your behalf.   

Where requirements are incomplete, please provide reasons. Your responses will help us understand your current stage and how we can support your progress.
