Healthwatch Lancashire want to hear about the lives of disabled people, what daily life is like and experiences using health and social care services in Lancashire.
Under the 2010 Equality Act, you are disabled if you have a physical or mental impairment that has a 'substantial' and 'long-term' negative effect on your ability to do 'normal' daily activities. This includes people who are not registered as disabled.
By sharing your experience, Healthwatch Lancashire will produce a report highlighting good practice along with any recommendations for areas of improvement. This will be shared with local decision makers to influence change and improvements for disabled people.
All responses will be anonymous. To protect your anonymity, your individual views will be combined with those of other respondents and analysed at a combined level. All information given will be held by Healthwatch Lancashire and remain secure and confidential, and will be used in line with the regulations of the Data Protection Act 2018. We will not pass your details on to any third parties, and it will not be used for marketing purposes.
