Background
This survey is for users of respite services (or their carers, friends or family). Respite services provide short breaks to service users, either one day or longer, usually away from your home.
How to complete this survey
a) Please refer to the information relating to this consultation on the consultation web page

c) Other questions will require one answer, for others you can select multiple answers
d) For some questions you need to click in the box and type in your answer
