Glasgow City Health and Social Care Partnership (HSCP) is now seeking nominations for individual staff, staff teams, project teams and volunteers for its HSCP Staff Awards for Excellence. Nominations must work within Glasgow City HSCP, although the ‘Volunteer of the Year’ category is also open to volunteers outside the HSCP organisation.
Please complete the following nomination form in full - all questions must be answered to submit a nomination and completed in one session; they cannot be saved and completed later.
When completing your nomination, please provide specific examples where possible. This helps the judging panel understand the impact and achievements of your nominee.
Guidance for making a nomination has been communicated by an email announcement to all Glasgow City HSCP staff. The guidance note includes the questions to the nomination form, as well as the award categories.
