Persistent pain (also sometimes called chronic pain) refers to pain that last beyond 3-6 months. Persistent pain can affect one or more areas of the body and may be widespread affecting many different areas. There may not be a clear underlying cause, or the pain and it’s impact is out of proportion to any underlying injuries or disease.  Often persistent pain does not resolve with physical or medical treatments and can cause distress and disability. The Community Persistent Pain Service offers the chance to explore strategies to live a more meaningful with pain rather than a cure.

Your NHS Number

Before you start, please ensure that you have your NHS Number at hand. Please note that we will not be able to process your referral without it.

Alternatively, you should be able to find your NHS Number on any letter or document you have received from the NHS, including prescriptions, test results, and hospital referral or appointment letters. Each NHS Number is made up of 10 digits shown in a 3-3-4 format (e.g. 000-000-0000). If you cannot find your NHS Number at home, you can ask your GP practice to help you.
