Dear Athlete

    We are inviting you to participate in a research study that we are conducting in the School of Sport, Exercise and Rehabilitation Sciences at the University of Birmingham that aims to understand performance in sport. You have been invited as you are aged sixteen years or over and compete in sport. There are no other criteria for participation.
    Participation in this study involves completing a questionnaire that asks some questions relating to your thoughts, feelings and actions in sport. The survey will take about 20 minutes to complete. All answers will remain confidential and anonymous. In other words, you cannot be identified at a later time. This means that once you complete the study you are no longer able to withdraw from the study. The results of this study may be used in future reports such as academic journals and conference presentations. Participation in the study is entirely voluntary.
   The information gained from this study may go some way to helping us understand the behavior of athletes. For this research to be useful, it is important that you respond honestly to all questions. Many thanks for your cooperation.
