Please complete this survey to arrange an Organisational Health surgery with Tonia Collett, Collett Consultancy. The surgeries are designed to offer light touch support on a topic you have identified. It will not be possible to use the surgeries to request intensive organisational health support, or repeat support sessions. If your needs exceed the offer of a surgery please contact Helen Sharp, Museum Development Programme Officer to discuss other options.  

MDM will acknowledge your request within two working days, and a member of the team will be in touch to arrange a time to meet within a week.

MDM will need to share the information you have provided on this form with Collett Consultancy for administrative purposes and to help Tonia Collett prepare before you meet.
