When we feel unwell, most of us would consider going to our GP or (in an emergency) a hospital for treatment. But did you know that there are lots of other services funded by the NHS and local authority to help you stay well?
You can go to most of these services directly, without referral from a health professional. This is known as 'self-referral'.

At Healthwatch Derbyshire, we often signpost people to self-referral services.
There are too many self-referral health and care services for us to list all of them, but what we would like to do is:
- Raise awareness of some of the services we tell people about most often
- Find out whether you have used these services and what your experience was like
- Hear your thoughts about how you would like to get information about these services.
The survey has 6 questions about self-referral services and should take only 2 - 5 minutes of your time.
There are some further questions about yourself. These questions are optional, but they help us understand how different groups of people experience health services.
