Healthwatch Northumberland Stellar Awards celebrate the people and services who provide a particularly positive health or social care experience for people in Northumberland.
This could be:
- An individual – either a professional or a volunteer;
- A team of people - for example, a hospital ward or a care home;
- A service - for example, ambulance service, a health or care support service
They could be working or volunteering in any NHS health or social care service within Northumberland, for example:
- GP, pharmacy, dental and eyecare services
- Community health/care services such as physiotherapy, mental health services
- Hospitals, ambulance/transport services
- Care Homes or home care support
- Patient support or social prescribing services
You can nominate a person, team or service for an award provided:
- They are not related to you.
- They are providing care or support within Northumberland.
- You are living in Northumberland (or did when you received care)
- You received the care in the last 12 months
- You read our Terms & Conditions below then provide all the details requested on the form that follows.
Terms and Conditions - please read:
- The nominee should be an individual, team, or service providing publicly funded care in Northumberland.
- Nominees should not be related to you.
- Nominations from health or social care professionals made in a professional capacity will not be accepted.
- Healthwatch Northumberland may name your nominee and your nomination reasons in promotional materials. We won't publish your name or an individual nominee's name without your/their consent.
- Healthwatch Northumberland is not responsible for lost, delayed, or incomplete nominations submitted in any format.
- Winners are chosen at the discretion of Healthwatch Northumberland based on the feedback received and decisions are final.
- A nomination does not mean Healthwatch Northumberland endorses the person, team, or service.
- Healthwatch Northumberland may change or withdraw the nomination programme at any time.
- Healthwatch Northumberland may update these terms and conditions without notice.
