About the Survey
We want to hear about your experience, or the experience of someone you care for, when leaving hospital. This includes going home or to another place of care.
The NHS wants to improve people’s experience of leaving hospital. Your feedback matters!
Every three months, one person who completes the survey will have the chance to win a £25 shopping voucher.
You must be 18 or older to complete this survey. You or the person you are completing it on behalf of must be a Derbyshire resident.

The information from this survey will be shared anonymously with the NHS Derby and Derbyshire Integrated Care Board (ICB) by Healthwatch Derbyshire. It could also be shared with NHS England - Midlands, Derbyshire County Council, and Derby City Council. It may be published as a report and shared online.
As well as your feedback, we also ask you to provide some personal information. You do not have to provide this information if you don't want to. If you choose to leave your personal information, this will be shared with the NHS for them to contact you and learn about your experience.
