Healthwatch York is the independent champion for people using local health and care services. We listen to people's experiences of local health and care services and share these views with those providing or commissioning the services to help improve them.

As part of this, our fully trained volunteer team is visiting local care and nursing homes to see them in action and speak to residents. As part of this, we also want to hear from residents' family and friends to understand what you think and your experience.

We will be visiting South Park care home on Friday 23 May. You are welcome to talk to our volunteers then. However, if you are not able to visit on that day, we still want to hear from you and have developed this short survey to capture your views.

All your feedback is anonymous and it will be part of a report summarising our visit and comments from family, friends and staff members as well as residents.

By completing this survey you agree to:

  • Your anonymous feedback being used and stored by Healthwatch York.
  • Voluntarily participate

1. Do you feel the home is clean and comfortable?

2. If no, please explain further:
