How the information will be used?
The information collected by the British Dental Association will primarily be used to inform future discussions and policy work on CDS Dentistry in Wales. All data will be used for research purposes only and any information you provide will be treated confidentially. All data will remain anonymous.
What happens to the data collected?
Data from all participants will be coded, combined and analysed by BDA staff.
Storage of your personal data
All information you provide to us is stored on secure servers. The data that we collect from you will not be transferred to, or stored at, a destination outside the European Economic Area ("EEA"). Your personal data collected through this survey will be stored for up to seven years. Data will be stored on our servers and our survey platform which is SmartSurvey.
Access to information
You have the right to request a copy of the information we hold about you.
What do I need to do?
You are not required to take part in this study but your participation will allow us to gather valuable evidence for future work. Your information will be aggregated with the other respondents' information. All responses will be anonymous.
The data controller
For the purpose of the General Data Protection Regulations 2018, the data controller is the British Dental Association of 64 Wimpole Street, London W1G 8YS.
The data processor
For the purpose of the Regulations, the data processors are both the British Dental Association and SmartSurvey Ltd of Unit 23, Basepoint Business Centre, Tewkesbury, GL20 8SD.
