Neurodisability Feedback Survey
We would like to understand what clinic appointments are like for young people and parents. To do this, we are using a short survey called the Experience of Service Questionnaire to gather feedback. We will be asking everyone who comes to a clinic within a set time in May to complete a questionnaire.
To help more families take part, we have created posters and flyers that will be displayed in the hospital and given out at clinics.
For this survey, we would like to get your feedback on:
- The questions we plan to use
- The posters and flyers we've made
Our aim is to make sure the questions and posters are helpful and accessible for everyone. We’d love to hear what would feel most meaningful or helpful to you.
This short survey should take 5 to 10 minutes and all of your answers will be anonymous.
