Welcome to our survey!
It should take you around 5-10 minutes to complete, depending on how much you wish to share.
We’d love to hear from you if you are:
✨ Aged 11–25
✨ Identify as a girl, woman, or gender diverse person
✨ Autistic or ADHD — diagnosed or self-identified (you don't need a formal diagnosis)
✨ Living or accessing support in Gloucestershire
If you don't meet the criteria above, we would still really love to hear about your experiences! You can contact us by filling out a form on our website (click here) or pop us a message on Insta or Facebook - just search Healthwatch Gloucestershire.
This survey is part of a Healthwatch Gloucestershire project exploring the experiences of girls, women, and gender-diverse young people (including trans and non-binary people). We want to understand what life is like in health, education, and community settings so we can share your voices in a public report that helps improve local services.
Your answers are anonymous — we won’t ask for your name or anything that can identify you.
The more detail you can share, the more we can understand your experiences. But there’s no pressure — only answer what feels comfortable. You can save and come back later if you need a break (you’ll just need an email address to do that).
Please note this survey closes on Monday the 8th December 2025.
Prize draw: There is an opportunity to leave your contact details at the end of the survey to enter a prize draw to win a £20 voucher. This will be used for the purposes of the prize draw only and not for any other purposes.
Contact Information: If you have any questions or need assistance, please contact our project team at info@healthwatchgloucestershire.co.uk or call 0800 652 5193. We will do everything we can to help you.
Thank you for helping us try to make Gloucestershire’s services better 💛
