In this survey, we are asking for your comments on a number of proposals relating to the cost of care services provided by Liverpool City Council.
Please respond to all the questions in the survey.
The survey is anonymous. All comments and responses will remain confidential and used only to help us to decide whether to retain the current system or implement the proposed changes.

Who is this survey for?
Anyone who receives adult care services arranged by Liverpool City Council will receive a survey in the post.
However everyone is welcome to complete the online survey.
Please complete the survey only once per person.
