Welcome to the survey about out-of-pocket costs related to diabetes.

The purpose of this anonymous survey is to understand the individual costs related to diabetes amongst a sample of adults living with diabetes in Ireland. 

This anonymous questionnaire asks questions about you, your diabetes and your quality of life. It also asks about your use of and payments made for health services, your work status, and transport relating to appointments, over the last 12 months.

We appreciate you taking the time to complete the questionnaire. There are no right or wrong answers.
