We’re using it to help the NHS North East and North Cumbria Integrated Care Board (NENC ICB) check how good Durham and Darlington NHS Foundation Trust (CDDFT) Children's Occupational Therapy Service is, and to learn about your experiences both the good and the bad.
If you or your child has accessed support from the County Durham and Darlington NHS Foundation Trust Children's Occupational Therapy service in the last 24 months, and you live in Darlington please complete this survey with your most recent experiences in mind.
This survey is for parents/carers of children aged 0-11, and for young people aged 12-18. If you are a young person aged 12 or over, you may wish to complete this survey yourself, or with support from your parent or carer.
The survey should take less than 10 to 15 minutes to finish.
Your answers will help us to make the service better. If there are any questions you don’t want to answer, you can leave them blank. If you or your child or young person have finished treatment and are no longer with the service, please answer the questions as if you were still getting help.
Your answers are anonymous, which means no one will know who you are. This will not change the care you get.
