This survey is to get your feedback on our Tackling Inequality Strategy. 

Your responses will be used to help us develop this strategy going forward.  We’ll only use them, including personal data such as your ethnicity anonymously, to help better understand the results of this survey. 

Please be advised that you may be identifiable if you include any personal details, such as names or email addresses in your answers. We will not publish or share any information that might identify you. We may use anonymised quotes of what you tell us when we review our findings from this research.

The information you provide will be stored securely by Diabetes UK and Smart Survey, the survey platform we use. We will delete all the responses from Smart Survey at the end of the project.
