Betsi Cadwaladr University Health Board is your local NHS. We provide the health services that you, your family and friends will recognise in your local area such as GPs, dentists and hospitals.

We are running a series of short surveys to gather your views and insights on key health topics. Your feedback is vital because it helps us understand what matters most to you and your community. By sharing your thoughts, you will help shape future health services and ensure they meet local needs. All responses will be reviewed and used to inform planning, improve services, and guide decision-making across the Health Board.

The first survey in this series will focus on prevention—how we can work together to try to shift from treating illness to preventing or reducing the risks of illness before it starts. Your input will help us design better services to support lifelong health and wellbeing.  This survey will take approximately 15 minutes to complete, and all your responses will be kept strictly confidential and used only for the purpose of this engagement exercise. 

The questionnaire is voluntary and confidential, all feedback received will be logged, reviewed and key themes identified. 

Please ensure that when you complete free text boxes, you do not enter any information, which may potentially identify you or any other individual.  The information you provide will be used to develop our develop our future health services and kept in line with Data Protection legislation.

Being healthy is shaped by a lot more than just what the NHS can do. The Health Board works with many different organisations to improve people's overall health and wellbeing. Are any of the following particularly important to you in terms of preventing illness? (tick all that apply).
