Health and Wellbeing Survey 2021

Thank you for taking time to help with this survey.  This survey has been created to understand your views on how you have been affected by Covid-19 and how you are feeling.

It should only take you 15 minutes or less to complete and your answers will be invaluable to Swindon Borough Council in helping us develop and evaluate our plans as the Covid-19 pandemic continues.

It is our hope that the survey results will help us to meet the needs of our community in the most appropriate ways. This will include our plans for social care, health services, employment, safety and general support in the community. In order to do this, we will ask you questions about your wellbeing, employment and experiences, including Covid-19 vaccination, during the Covid-19 pandemic. At the end of the survey, we will ask you to complete a section which includes some personal details, this enables us to tailor our response to the needs of our residents. This section is entirely voluntary. Please look at each question and select the response that you think is closest to the way you are feeling at the moment.

Your survey responses will be treated with strict confidence and the contents will not be shared with any third parties.  The survey outcomes will only be made public in ways that never identify those who took part, such as through anonymous statistics. More details of how your data will be processed, together with our Data Protection obligations and your Data Protection rights can be found within the full Privacy Notice for this survey by which is available here.

If you have any queries about the survey please contact us using this telephone number 01793 465513 or email address PublicHealth@swindon.gov.uk.

The fully anonymised results of the survey will be publicised and shared on the Swindon Borough Council website in 2022. The results of the 2020 Health and Wellbeing survey can be found by clicking here.
