Section 19 of the Education Act 1996 places a duty on LAs to make suitable alternative education for children of statutory school age who cannot attend school because of illness, exclusion or 'otherwise'.

CLINICAL STAFF completing this form are reminded to provide evidence of the illness which precludes their patient from attending their current school. (Where it is agreed that education should be provided  then Alternative Education may be offered at another school or setting)  Following consultation in November 2022 with Devon Local Medical Committee it has been agreed that advice for children with long term health conditions will be accepted from Consultants, Specialists and Speciality doctors
