The Health Services Safety Investigations Body (HSSIB) came into operation on 1 October 2023. We have developed a strategy, to shape our future direction.
Following an engagement process where we asked stakeholders for their views, we have refined the strategy. We would now like to ask for your views on the revised strategy through this public consultation, to better inform the content.
In addition, we would like to ask for your views on HSSIB's investigation criteria. The criteria is designed to allow assessment across the available evidence, extent of risk and potential for improving care provision. It sets out the criteria and principles for HSSIB investigations.
