At NHS Test and Trace, we are keen to speak to members of households taking part in the Schools Self-Test Collection Trial.
Please fill in this form if you are happy for us to contact you to offer you the opportunity to take part in research related to the Schools Self-Test Collection Trial. The research is being conducted by NHS Test and Trace at the Department of Health and Social Care.
The research will include a 30 minute remote research call with you and/or short survey with members of household bubbles to understand more about yours/their views and experiences of collecting test kits from schools.
We are interested in speaking with members of household bubbles
who don’t opt to take part in home testing, as well as those that do. Please note, participation in this research is entirely voluntary.
You will be asked to provide your name and email address so we can contact you for this research. We will not use your personal information for any other purpose other than for research purposes, and we will not keep it longer than 12 months.

Do I have to take part?
Participation in this research is voluntary, and you will be formally asked for consent before any research session is arranged. You can change your mind at any time and withdraw from the research by emailing josh.avern@dhsc.gov.uk
