Kirklees Autism Reference Group - your thoughts

 
Kirklees Autism Partnership Board have identified a need to gain more input from autistic adults living in Kirklees. In order to do this they will be looking to set up a Kirklees Autism Information and Engagement Forum to gather the views on changes to services and what peoples priorities are.

We have developed a short survey which we would like you to complete. The survey aims to find out what you think are the best ways for us to develop the Kirklees Autism Information and Engagement Forum.

1. Are you autistic or are you a parent or carer of someone who is autistic?

 

2. Do you live in Kirklees?

 

3. Would you like to be part of the Kirklees Autism Information and Engagement Forum?

 

4. If "yes" to Q3, how would you like to be involved? E.g. attend in person, attend a virtual online group, etc.

 

5. If "no" to Q3, is there a reason why not?

 

6. Is there a particular day or time that would be best for either online or in person meetings? Please say in the text box below.

 

7. How would you like the meetings to work? E.g. a set agenda, each meeting to focus on a particular theme, meetings where you can raise any concerns, a mix of all the above, anything else? (Please tick all that apply)
