Autism Health Passport - Your Feedback

 
   

Leeds Autism AIM, part of the Advonet Group and Leeds Teaching Hospitals NHS Trust's Learning Disability and Autism Team have made a draft Autism Health Passport.

It can be used when going to a GP, hospital, other health and mental health service run by the NHS and other groups in Leeds. It is for autistic people in the Leeds area.

We worked on it with the Leeds Autism Diagnostic Service (LADS), Leeds and York Partnership NHS Foundation Trust, Leeds Mind and Leeds Mental Wellbeing Service.

The text-only version is 12 pages long. The Easy Read version is 20 pages long. To see both draft copies, please click on the links below to open both PDF documents:
Once it is done, the passport will be available as a downloadable Word document and an online form.

You can save it as a PDF and send it to the service you are accessing.

You can also have a printed copy of the passport and save it on your smartphone or tablet to show to services.
