Thank you for requesting a booking form for a place on a Living Well Programme course. Please complete this survey as soon as possible to reserve your place on the course.

Firstly, please select which Living Well Programme course you wish to book on. The next questions will determine your suitability for the course.

When you have completed the booking form, a member of the LWP team will be in contact in 5-7 working days (unless your course date is closer). This will be either by email or telephone call to the contact details you have provided - please be advised the LWP office telephone number will appear as ‘withheld’.

The LWP office is open Monday-Friday from 9am-5pm, excluding public holidays.

Each course is 6 sessions long and participants must attend the first session in order to progress through the course.

ONLINE COURSE: 10am -12pm, every Friday from Friday 30th January - Friday 13th March 2026 NB: no session on 20th February
HAYWARDS HEALT COMMUNITY COURSE at The Vale Surgery: 10am-12.30pm every Wednesday from Wednesday 25th February - Wednesday 1st April 2026- REGISTER INTEREST
