We are conducting this survey to better understand how mobility challenges affect health events such as falls, ambulance call-outs, hospital admissions, GP visits, pressure sores, and infections among people living with Motor Neurone Disease (MND). This survey is intended for people living with MND. You can also ask someone else to help you fill it out. Your responses will help inform advocacy, service planning, and support provided by the MND Association. This survey should take approximately 10–15 minutes to complete.
