This form is intended for people who would like to apply for a place on Artlift’s Dance Translations course. (Healthcare or other professionals wanting to make a referral, should use our standard Chronic Pain or Create Well Mental Health forms, which are available on our website).

INFORMATION & CRITERIA:
Dance Translations offers non-clinical group courses in community venues, which can sometimes be accessed remotely (via Zoom).
Sessions are free, and all resources are provided. No dance skills, talent or special equipment are necessary. Visit our website to find out more: www.artlift.org.
Our Dance Translations courses offer:
- A chance to develop your own movement possibilities, to get creative and meet others in courses of 8 x 1.5-hour group sessions.
- Creative Check-ins (one-to-one conversations or video calls) with a specially trained Dance Facilitator to help you explore, embed and sustain creative movement activities.
- Move On plans, so that you can continue to use dance as a tool to support ongoing improved wellbeing.
Our Dance Translations courses are for adults who are:
- Over 18 and registered with a GP in Gloucestershire
- Living with persistent, debilitating pain for over 3 months and / or
- Living with mental health challenges
- Experiencing significant emotional distress and/or interference in daily life due physical and/or mental health issues
- Comfortable in a group setting
- Able to follow simple instructions or demonstrations
- Committed to attending as many of the sessions as possible
