This Preston, Chorley and South Ribble Maternity and Neonatal Voices Partnership survey is to request your feedback about how we should engage with families who have had an experience of using neonatal intensive care services, either within Lancashire Teaching Hospital Trust (Preston NICU) or elsewhere.
We'd like to know your thoughts about how we should listen to you, so that we can better represent your opinions in the work we do with the Maternity and Neonatal teams, Health Visitors and the wider services that support you, and so you can get involved in coproduction and feedback events in a way that is right for you and your family, and at a time that feels appropriate.
We recognise that a neonatal journey may be traumatic, and that by completing this survey or being involved in coproduction work you may have upsetting memories triggered which could be difficult for your and your family to deal with. If you would rather not be involved, please exit the survey now. If you do want to proceed, you can leave the survey at any point and your contributions will still be valued. If you need support to help overcome the trauma you may have experienced, you can seek support from Bliss, numerous charities at North West Operational Delivery Unit, you can request a debrief of your birth from birthafterthoughts@lthtr.nhs.uk, Telephone (answer machine): 01772 524485 or if you would like to speak to trust or the MNVP directly please request contact from us.
You can remain anonymous if you wish, but if you would like to be contacted by Preston, Chorley and South Ribble Maternity and Neonatal Voices Partnership to discuss your feedback, or if you are interested in providing a more detail about your experiences, please leave your contact details below.
Thank you!
