NHS Lanarkshire - Living with Multiple Long Term Conditions Survey

The purpose of this survey is to learn from the lived experiences of people living with two or more long term conditions in order to help us better understand the impact that they have on your life. We are also keen to understand the impact that the COVID-19 pandemic has had on you. Your views will help us to shape health and social care services so that they are better able to meet your needs.
