In the last 12 months, have you, or someone you wish to represent, been referred to a specialist for tests, diagnosis or treatment by your GP practice? If yes, please help us improve the referral process for everyone. This will only take 5-10 minutes of your time.

Prize draw: There is an opportunity to leave your contact details at the end of the survey to enter a prize draw to win a £25 voucher. This will be used for the purposes of the prize draw only and not for any other purposes.

A GP practice referral is when a GP or other medical professional at a GP practice sends you to a specialist at a hospital or community clinic for further tests, diagnosis or treatment. This can happen during a phone or face-to-face appointment. Referrals could be for a hip replacement, MRI scan, physiotherapy, mental health services, endoscopy and more.

In this survey, we’ll ask you:

About your experience of trying to get a referral from your GP practice to a hospital or community clinic/ community service e.g. physio.

Optional information about you, including health conditions, ethnicity, sexuality, financial security and education. (If you don’t want to share this, you can answer “prefer not to say”). This helps us understand how different groups experience health services, to reduce inequalities.

We’ll use survey responses to understand experiences and report to the NHS on how to improve the referrals process.
