A GP practice referral is when a GP or other medical professional at a GP practice sends you to a specialist at a hospital or community clinic for further tests, diagnosis or treatment. This can happen during a phone or face-to-face appointment. Referrals could be for a hip replacement, MRI scan, physiotherapy, mental health services, endoscopy and more.

Optional information about you, including health conditions, ethnicity, sexuality, financial security and education. (If you don’t want to share this, you can answer “prefer not to say”). This helps us understand how different groups experience health services, to reduce inequalities.
We’ll use survey responses to understand experiences and report to the NHS on how to improve the referrals process.
