Who is this survey for?
This survey is for parents, carers, and guardians of young people with special educational needs and disabilities (SEND) who are in school years 9 – 13, or aged 13 – 20. It asks about the support young people need as they move toward adulthood.
If you support more than one child or young person with SEND, please answer the survey for just one of them. This helps us to understand your experiences more clearly, as parents may have children at multiple different stages or with different needs.
You can fill in the survey again for another child or young person if you want to, but you do not have to. If this isn't possible, you can use the free text boxes to describe your families experiences more broadly.
Who is asking the questions and why?
We are Healthwatch Suffolk. We listen to people’s views about using health and social care services. We are an independent organisation, and we use what people tell us to help make local services better.
We work with Suffolk County Council, the NHS and local charities. Your answers will help them understand what works well for young people and families of young people with special educational needs and disabilities (SEND) in Suffolk and what could be better.
Do I have to take part?
No - taking part is your choice. You can leave any question you do not want to answer.
You can save the online survey and come back later. Click “save and continue” at the bottom of the page and enter your email address to get a link.
What will the survey ask about?
The survey asks questions about your child or young person and the support they get as you grow up and prepare to be an adult. This includes:
- Information about their special educational needs or disabilities.
- Their education and planning for their future
- The support they get to move from children’s to health or social care services.
- Building life skills and independence.
- Their community.
What happens to my answers?
The survey is anonymous. This means we will not ask for your name, and we will not know who you are.
Only the Healthwatch Suffolk team will see all your answers.
Your answers will be put together with answers from other parents, carers and guardians. We will share a report about everyone’s answers with:
- The public
- Suffolk County Council
- The NHS
- Local charities
We may use short quotes from what you write. We will do our best to keep you anonymous, for example removing names, dates or the names of services like schools or health care. But please be aware there is a small chance someone could recognise you from your experience.

If you have any questions, or need more help, you can contact Healthwatch Suffolk. By contacting us, you can:

- Take part in the survey over the phone with our team.
