Introduction
The Posture and Mobility service is reviewing and evaluating the clinical environment and facilities provided and you are being invited to take part in this work. Before you take part, it is important that we help you to understand the purpose and nature of the service evaluation and what your participation will involve, if you take part.

Please read the following information carefully, and please get in touch if anything is not clear, or if you want more information. Contact details are given at the end of this information sheet.

What is the purpose of the review and how will it be carried out?
The purpose is to identify the expectations in terms of environment and facilities and to identify how these can be improved to meet current and future expectations and needs.

Why have I been invited to take part?
The aim is to invite participants who have experience of current environment and facilities provided by the Posture & Mobility service who are able to provide insight from an individual's perspective.

Do I have to take part?
Participation is entirely voluntary. It is up to you to decide whether or not to take part.  Some personal data is gathered for equality monitoring purposes - all other data should be provided anonymously for this survey - you should not share any information that will identify you or a member of BCU staff.

What will taking part involve?
Taking part will involve answering a short survey which will take approximately 5-10minutes.
The questionnaire is voluntary, anonymous and confidential, all feedback received will be logged, reviewed and key themes identified, and will be used to help inform any future options development and decisions about improvements in the Posture and Mobility Service.


Will my participation be confidential?
All information about you collected during the review will be kept strictly confidential and stored securely in accordance with the Data Protection Act.
Only BCU staff will have access to this information and any data gathered will be used for the purpose of gathering insight and evaluating the Posture and Mobility Service.  All data, whether electronic or paper or in any other form will be kept in accordance with Data Protection Act and will not be kept any longer than is necessary to complete the service evaluation.

What will you do with the results of the research?
Data collected from the questionnaire will be analysed and will be incorporated into service review. No participant will be identifiable in the review.
