We would love to hear feedback about your experiences and any challenges or barriers you may have faced when accessing sight (eye) tests as part of a Black or Asian minoritised community. 

 

Healthwatch Surrey champions the voice of local people to shape, improve and get the best from NHS, health and social care services. We are independent and have statutory powers to make sure decision makers listen to the experiences of local people.

 

Healthwatch Surrey is seeking to understand the barriers that may prevent people from Black or Asian minoritised communities from accessing sight (eye) tests and we would like to hear about the experience.

 

Responses to this survey are anonymous. The closing date for the survey is 28 November. 

5 people who complete the survey before 28 November will be selected at random to receive a £10 'One 4 All' gift card. Those selected will be notified within 1 month of the close of the survey.

There are up to 23 questions (plus 12 optional demographic questions) and it will take about 15 minutes to complete.
