We want to learn about your smoking habits and reasons why you smoke.
We also want to know what might make you want to stop smoking and if you know about the stop smoking service in Derbyshire or Derby City.
This will only take 5-10 minutes of your time, and your answers will help services provide better support to people in the future.
Most questions are optional.
At the end of the survey, you can choose to enter a prize draw to win a £50 shopping voucher.
