Healthwatch is the independent champion for people who use health and care services. We are working with providers of health and social care services to help them understand your experiences.

We want to understand how having a stroke has affected your health, daily life, and overall wellbeing. We are also interested in hearing about your experience of the care you received, as well as any ideas you have for how services could be improved.

You can also complete this survey on behalf of someone who has had a stroke.

Closing date: Tuesday 30 June 2026
Estimated time to complete: 10 minutes

Your feedback will be shared anonymously with local NHS decision-makers to help them understand people’s experiences and improve services across Calderdale and Kirklees.

All information you share is anonymous and will be stored in line with data protection legislation. To find out more about how we use and store information, please see our privacy statement

If you have any questions, need support to complete the survey, or would prefer to share your experiences in a different way, please contact us:
